Flying a Cat Through Continental Airlines Through US., what do i need to do?

I am not planning to have the animal in the cabin but sent with the cargo (luggage). Do I need a health certificate from a vet? How much does continental charge? I have been calling them but i dont get an answer and the continental website does not answer these questions. Have you had the experience of traveling with a cat or dog through continental? Tell me about it, please. I already read that Charlie and it does not answer my questions.

2. Uhm....Charlie gave correct info and it DOES answer your questions. His link shows what paperwork you need, kennel/crate requirements, when/where you are to arrive with the animal as well as the phone number for the Live Animal Desk where they give you the rates among other things..
