2. Alaska is flying to a lot of major cities across the US (Chicago, Newark, Boston, Miami, Orlando, DFW, Washington DC, and Minneapolis in October), including Hawaii now. Flights to Mexico and Canada too.
3. It is a better bet to book online. US Airlines are doing bad financially so they charge for everything. If you book by the phone you might have to pay more. Happy Travelling PS: US Airlines are pretty horrible compared to carriers from other countries. Asian Airlines tend to be the best.

6. Also: Horizon Frontier Aloha (only to and within Hawaii) Cape Air (only in Indiana and Massachusetts) To get any of their phone numbers you can look on their website, or call 1-800-555-1212 and ask them for the toll-free number of the airline. This call is free, also.
